St. Thomas Gets Grant For Patient Care Program

NASHVILLE, Tenn. – (April 5, 2016) – Saint Thomas Medical Partners (STMP) Family Health Center- West, located at 5201 Charlotte Pike in Nashville, has received a one-year $45,000 grant from Baptist Healing Trust.  The grant will fund a Patient Care Navigator Program which will employ a full-time Patient Care Navigator (PCN) whose purpose is to work with uninsured patients and patients with multiple diagnoses or chronic conditions.  The navigator will link patients with accessible medications, medical supplies, transportation and community resources for improved health outcomes.

            The Family Health Center – West is part of the Saint Thomas Health’s network of family health practices serving the community by providing health care to all ages regardless of insurance or financial resources.  Saint Thomas Health is part of Ascension, the nation’s largest Catholic and non-profit health system.

            The PCN works as a case manager for poor and vulnerable patients who have been referred to specialist physicians at Saint Thomas Midtown and Saint Thomas West Hospitals.  Caring for the poor is a key focus of Saint Thomas Health’s mission.  Navigators help patients connect with physicians and other health care providers to obtain the medical care they need, complete paperwork, and access community resources such as housing, food banks, clothing and transportation.

            The Baptist Healing Trust, which recently awarded more than $1.4 million in its current grant cycle, is a private grant-making foundation whose mission is the sacred work of fostering healing and wholeness for vulnerable populations through strategic investing, philanthropy and advocacy.  Recipients of contributions from the Baptist Healing Trust include non-profit organizations from 40 counties in Middle Tennessee that focus on advocacy, physical health, mental health, recovery from alcohol and drug abuse, or healing from abuse, neglect and violence.  This year’s grants bring the collective grant-making of the Baptist Healing Trust to more than $73 million since 2002.  The Trust, while created out of the sale of Baptist Hospital to Ascension Health/Saint Thomas Health Services, is a private, independent foundation and is not affiliated with Saint Thomas Health Services or the Saint Thomas Health Services Fund.
